GUIDELINES FOR CANDIDATE'S SELF-STATEMENT

SUMMARY OF ACTIVITIES

[Not more than 3 to 5 pages]

A faculty member who is eligible for advancement or for whom an appraisal review is to be held is asked to provide to his Chair an informal essay summarizing the important aspects of his work. The essay should provide information which is not listed in the SER and should explain candidate's philosophy of teaching, construction of new courses, the nature of collaborative research, etc. Attach a copy of any Sabbatical Leave Report for leave(s) taken since your last review.

Information should be provided on the following categories of activities:

Teaching

  • Important aspects of course development and revision
  • Development of new and effective techniques of instruction
  • Analysis of the scores received in the student evaluation surveys

 

Research and Creative Work

  • Brief description of research activities (current and proposed)
  • Recognition received for significant and original research contributions

 

Joint Authorship

Provide a brief description of the nature of collaborative research.

Attach a separate listing of UCLA faculty who have co-authored publications with you.

If you have not individually labeled co-authored publications listed in the Bibliography of your

Summary of Experience Record (SER), provide a statement describing the nature of your co- authored works.

Brief description of significant professional activities

  • Activities Service to scholarly and professional societies
  • Consulting and community

 

University and Public Service

Brief description of participation

  • on department and University committees
  • on student-faculty committees
  • as adviser to student organizations
  • in scholarly services to public agencies
